Privately funded lunatic asylums were widely established during the nineteenth century. The incidence and prevalence have both increased tremendously in the background of growing insecurity, terrorism, economical problems, political uncertainty, unemployment and disruption of the social fabric. By bringing together different agencies from health, social care, housing, education, employment, and benefits, people with mental health problems are more likely to be able to access the services they need to lead a normal life. What does it mean to talk about ‘mental health in historical perspective’? At the micro level, however, a tighter focus on institutions can reveal countless stories about the lives of the hundreds of thousands of patients and staff for whom the asylum was either home or their place of work. Although we may be more able to treat the symptoms of some disorders with drugs and other therapies, the complicated causes of mental illness remain elusive. The first is a brief introduction to various criteria we use to define or distinguish between normality and abnormality. Institutional care dominated psychiatric practice for approximately 150 years in many parts of the world. There is growing recognition in the fields of public health and mental health services research that the provision of clinical services to individuals is not a viable approach to meeting the mental health needs of a population. Psychiatric interventions occur in mental hospitals, psychiatric wards in general hospitals, special hospitals, medium secure units, day hospitals, outpatient clinics, child guidance clinics, prisons, children’s homes, sheltered housing, drop-in centres, community mental health centres, domiciliary care by community psychiatric nurses, multiple forms of psychological therapies, and, of course, in the general practitioner’s surgery, not least through the increasing prescription of psychiatric drugs. Often deploying the tools of self-help, assisted by the revolution in communication brought about by the internet and by a greater openness in talking about mental health, the new approach was attractive as a way to overcome the dual problem that had always held back an expansion of mental health services: the inadequacy of both funding and expertise.
